Inpatient Referral Process
If you are in an acute-care hospital, you and your family should discuss your interest in coming to Drake Center with your attending physician or social worker/discharge planner. You will need a referral . This can be done by calling 513.418.4365 or 1.800.948.0003 x 4365. Referral sources may also visit the For Physicians or For Discharge Planners sections of our web site for additional information.
If you are at home and are interested in coming to Drake Center for extended rehabilitation, call our referral line at 513.418.4365 or 1.800.948.0003 x 4365 to begin the process. If you are not currently receiving therapy, you will need to be evaluated by a Drake Center physician through our Outpatient Clinic before we can begin the admission process.
Once we receive the referral:
- One our assessment nurses will provide a free patient evaluation to determine whether or not you meet the criteria for admission and also to speak with you and your family.
- If you meet the admission criteria, our assessment nurse will contact your insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for a transfer to Drake Center.
- Once approved, he/she will notify the referring hospital's discharge planner (if applicable) and work with our admitting office to coordinate the transfer/admission.
Benefits of early referrals
If you or a loved one is coming to Drake from an acute-care hospital, a key to being admitted to Drake is getting an early referral from your physician. In doing so, the likelihood of meeting our criteria for long-term acute care is much higher. Unfortunately, patients who fail to meet our criteria often do so because they are too advanced in their recovery.
