Outpatient Referral & Registration
Outpatient services are those provided to patients or individuals not requiring inpatient stays. Drake's complete range of outpatient care - including rehabilitation and wellness services - allows a variety of treatment options in a more cost-effective setting. It also helps you continue working toward your highest possible level of functioning and independence. You may begin our outpatient program directly after your discharge from Drake Center or another hospital, or you may begin at any time you can benefit.
Outpatient services include physical therapy, warm-water aquatic therapy, occupational therapy, speech-language pathology, cardiopulmonary rehabilitation, spasticity and mobility management, neurogenic incontinence management, multiple sclerosis management, lymphedema management, wound management, spine management, community reentry, neuropsychological examinations, stroke recovery and diagnostic radiology services.
Our outpatient services are conveniently located at our main campus in Hartwell with free parking and easy access. Drake Center also offers outpatient services in Clifton and West Chester.

To receive outpatient services, you will need a doctor's referral/prescription. Physician orders may be faxed directly to our Outpatient Department (513.418.2550) or brought in with the patient.
You will need to bring the following with you on the day of your outpatient registration:
- Picture ID
- Insurance, Medicare or Medicaid identification card
- Co-payment (if applicable)
- Doctor's order (if not faxed in advance)
