Our Transitional Skilled Nursing Care (TCU) provides short-term skilled medical and rehabilitation care to bridge the gap between hospital-level and a more independent level of care. It combines our expert physical rehabilitation and the high-quality medical/nursing services of our skilled nursing facility for specific patient populations (i.e., geriatric orthopedic rehabilitation).
TCU clinical admission and continued stay criteria
- Medically Stable
- Age > 18
- Anticipated length of stay less than 60 days
- Rehabilitation potential with expectation for clinical/functional improvement, patient willing to participate in therapy program
- Full thickness wound/pressure ulcer stage II - IV, wound care interventions < 1hr/day, minimum 3x/week. Negative Pressure Therapy.
- Stable tracheostomy management and/or decannulation services.
- Respiratory treatments and interventions, nebulizer treatments, oxygen therapy adjustments, suctioning not to exceed Q4 hour interventions.
- Isolation, active infection with C Difficile, MRSA/VRE/MDRA
- IV Anti-infectives < 2 drugs/ < 4x/24 hours (maximum Q6 hrs)
- IV Fluids via PICC, peripheral line
- Ostomy management and training for self care
- Nutritional management/tube feeding via G tube/PEG
