Transitional Care Unit

Drake Center offers transitional skilled nursing care designed to provide short-term medical and rehabilitation services for specific patient populations. Our Transitional Care Unit (TCU) is built on Drake Center’s established expertise in physical rehabilitation and extended medical care services. This unit will serve patients as the bridge between the acute care hospital and their return to the community.

Drake Center launched the TCU with its Orthopedic Rehabilitation Program, which serves geriatric patients being discharged from the acute care hospital after orthopedic surgical procedures or fractures requiring therapeutic treatment. Its primary goal is to provide therapeutic intervention that will maximize each patient’s functional abilities and activities of daily living. The appropriate patient will be medically stable and benefit from at least 60 minutes of rehabilitation services per day, to include an individualized program of physical and/or occupational therapy and possibly other therapies as needed. The average length of stay for most patients is 14 days.

An internal medicine physician leads our TCU treatment team, which will also include nurses, physical therapists, occupational therapists and social workers. In addition, a physical medicine and rehabilitation consult is available when required, as are consults by speech therapists and recreational therapists. A dedicated therapy gym is located next to the unit.
