After Your Discharge

Outpatient services

Your discharge plan may include outpatient services. Drake Center’s interdisciplinary outpatient rehabilitation team helps individuals continue progressing toward their highest possible levels of functioning and independence.

If your physician recommends these services, your case manager can coordinate them for you. For details, see the Outpatient Services section.

Patient survey

Shortly after your discharge, we will mail you a patient survey to assess your satisfaction with Drake Center services. Please take a few minutes to complete it and mail it back. Our goal is to provide the best possible care to our patients, and with your help we will continue to do so.

Assisted living on Drake Center’s campus

Bridgeway Pointe, on Drake Center’s campus, is unlike any other assisted-living facility in Greater Cincinnati. For adults of all ages, it offers the comfort of apartments with comprehensive health, wellness, and rehabilitative services. Community support groups

Meeting with others who face similar challenges can be invaluable in coping with life after discharge. Having a safe place to meet, share your concerns, and express your thoughts is vital for coping with the effects of disease and disability.

Community outreach

Drake Center offers conference center and meeting room space to health care-related organizations and support groups that meet the educational and resource needs of our community. If you are interested in having your group meet at Drake Center, please call 513-418-2544 for details.

Drake Center publishes a wide variety of community publications to inform community members about its services and activities. Some publications can be found on display racks around the facility.
